Forms & Other Documents
Occasionally, a driver may be required to provide a DOT medical examiner with additional information to confirm that they are regularly consulting with their primary care provider or specialist for a particular condition. This may also be necessary if they have been prescribed a new medication that could affect their driving or if they have a condition that requires close monitoring. Below, you will find common forms requested by DOT medical examiners, as well as an example letter of fitness to drive a commercial motor vehicle.
