What to Expect for Your DOT Physical Exam
1. Scheduling Your Appointment

Book your exam: Schedule your DOT physical here.

If your company is organizing: Follow the instructions in the email from your organization or RoadSafe DOT.
2. Before Your Exam

Confirmation email: After scheduling, you’ll receive a confirmation email with a link to 3bexam, our secure DOT exam platform.

Enter your information: Complete your details exactly as they appear on your driver’s license.

Access the portal: Once registered, you’ll have access to our secure, HIPAA-compliant online portal.

Complete driver history: Fill out the driver history section and sign the release form before your exam.
3. On the Day of Your Exam

Exam location: Your DOT medical examiner will arrive at your requested location and set up for the exam. No undressing or gown is required.

Bring your driver’s license: Have your valid, unexpired driver’s license ready. You can also upload a copy through the online portal.

Medical history: If you have a chronic condition or significant medical event (such as diabetes, high blood pressure, heart disease, sleep disorders, neurological conditions, etc.), bring your most recent provider or specialist note. A clearance letter from your provider is also recommended.

Medications: Bring a list of your current medications and the names/contact numbers of the prescribing doctors. If you take new medications or medications for pain, psychiatric conditions, ADHD, sleep disorders, or smoking cessation, have your provider complete the Driver Medication Form.

Sleep apnea: If you have sleep apnea, provide a compliance report from the last 30 days. If you may have sleep apnea, consider a sleep evaluation with a specialist.

High blood pressure: Avoid caffeine, energy drinks, tobacco, or stimulants before your exam. Take your blood pressure medication as prescribed. If your blood pressure is above 140/90, a repeat check will be done. Persistent elevation may require follow-up or limited certification.
4. What’s Included in the Exam

History & Physical: Review of your personal and family medical history.

Blood Pressure Check

Vision Test: Bring and wear your glasses or contact lenses if needed.

Hearing Test: Bring your hearing aids if you use them.

Urine Test: We check for pH, blood, hydration, and protein only. We do not perform drug testing as this is not currently required by the FMCSA. Be prepared to provide a urine sample, as any delays may result in additional fees.

Drug Test: Not routinely required, but may be requested if clinically indicated. If required, you must complete a chain of custody (COC) drug test within 45 days. New CDL hires must complete a COC drug test and have a negative result before driving which will be coordinate by your employer.
5. Exam Duration

Each exam takes about 15–30 minutes. Completing your online forms in advance will help keep your appointment quick and efficient.
6. After the Exam

If you meet FMCSA standards and payment is complete, you’ll receive your DOT medical certificate.

Certification length may vary based on your health history.
