Service Description

In some instances, you will be required to provide more information which may include additional documentation (i.e., medication form or clearance from specialist), additional testing (i.e., sleep study for suspected sleep apnea, abnormal vision, hearing or blood pressure screening). If granted a Determination Pending status, you will have up to 45 days to complete required requests from your medical examiner. A nominal fee will be applied for documentation completion and registering with federal database. Please note all determinations and certificates will not be issued until payment is made. A small service charge may be applied for long-distance requests. We accept Cash, Credit Card, or Venmo.
